Matrix Providers is hiring for a qualified Registered Nurse (RN) to join our team of talented professionals who provide health care services to our veterans and their families at David Grant Medical Center located on Travis AFB, Fairfield, CA.
• Status: Full time
• Schedule: Monday - Friday 7:30 am - 4:30 pm

The Registered Nurse (RN) Discharge Planning must have the following qualifications:
- Degree: Associates Degree of Nursing.
- Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) or as otherwise identified in the TO.
- Experience: A minimum of 3 years for HCW with an Associate’s Degree in Nursing/2 years for BSN, full-time experience in clinical case management or discharge planning for adults, children, families, seniors and groups.
- Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO.
